AGNES + Secret RF Eye Bag Removal: Why Clifford Clinic Uses a Two-Step Protocol
Many clinics in Singapore offer radiofrequency (RF) treatment for eye bags. Most use a single device. At Clifford Clinic, we deliberately use two — AGNES and Secret RF — in a structured two-step protocol that our doctors have refined over more than 10 years and over 500 eye bag patients.
This article explains what each device does, why the combination allows more accurate targeting than a one-device approach. I will also explain who is suitable and what to expect from treatment.
The problem with treating eye bags as a single-layer problem
A true eye bag is not just “extra fat”. It is a structural change involving at least two layers:
- The fat layer.Small fat pads within the orbit push forward through a weakened orbital septum, creating the visible bulge.
- The skin and support layer.The overlying lower eyelid skin must contract and re-drape smoothly once the fat pad is reduced. If skin quality is poor or untreated, the result can look deflated or crepey rather than refreshed.
A device that only reduces fat addresses the first layer. A device that only tightens skin addresses the second. Treating one layer and ignoring the other is the most common reason RF eye bag results disappoint. Our two-step protocol is designed to address both.

Step 1: AGNES — precision RF fat reduction

AGNES is a precision radiofrequency device that uses fine, micro-insulated needles. The insulation along the needle shaft means RF energy is released only at the exposed tip, at a fixed and controlled depth. In practical terms, this allows the doctor to:
- Treat the specific lower eyelid fat compartmentsresponsible for the bulge — patients often have protrusion in one, two or all three of the lower fat pads, and AGNES allows each to be addressed selectively.
- Protect surrounding structures, because energy is not scattered through the overlying skin or deeper tissues.
- Deliver energy where the diagnosis says it is needed, rather than treating the whole under-eye uniformly.
The RF energy causes the targeted fat to break down gradually. Over the following weeks, the body clears the treated fat naturally and the bulge flattens.
Step 2: Secret RF — skin quality and support

Secret RF is a cannula with a RF tip that delivers controlled RF energy across the eye bag fats. In our eye bag protocol, Secret RF is used to:

AGNES + Secret RF allows more accurate targeting. AGNES allows the doctor to melt fat from the outside in, through a top-down approach. Secret RF allows for the fat to be penetrated deeper but also more superficially as well, allowing for some skin tightening.
The skin around the eye is thin and delicate. Using a single device aggressively to compensate for what it cannot do well is exactly how complications and unnatural results occur. A two-step protocol lets each step stay conservative and controlled.
Who is suitable for the AGNES + Secret RF protocol?

In our experience, the ideal candidate has:
- Mild Grade 1 to 2 eye bags— early to moderate fat protrusion.
- Reasonably firm lower eyelid skin— the skin must be able to contract over the reduced fat pad.
- A genuine fat protrusion problem, confirmed on examination, rather than tear trough hollowing, mid-cheek deficiency or temporary fluid puffiness.
The protocol is generally not suitable for patients with Grade 3–4 eye bags, significant loose skin or lower eyelid laxity. In these cases, commonly in older patients, surgery such as scarless eye bag removal, sometimes with a pinch blepharoplasty to remove excess skin, gives a more complete and longer-lasting result. Because The Clifford Clinic offers both pathways, you will be advised on the option that fits your anatomy, not the option that fits a machine.
What the treatment is like
- Assessment: The doctor grades your eye bags, maps the involved fat compartments and checks skin quality. If suitable, then Dr Ee will advise for AGNES + Secret RF eye bag removal.
- Numbing: Optional topical numbing cream for about 30 minutes. Local anaesthesia will be injected before the procedure
- Procedure: Approximately 20 minutes for the AGNES + Secret RF treatment itself.
- Recovery: Most patients experience around 4 to 5 days of mild bruising and swelling. Temporary dry eyes may occur for 1 to 2 days. Occasionally a small, non-visible firmness can be felt under the eye as the treated fat breaks down. This settles with time and is monitored at your review.
- Results: improvement may be visible from about 2 weeks, with full results over 1 to 2 months as the fat clears and collagen remodels. Results typically last 3 to 5 years, though natural ageing continues.
Pricing
The AGNES + Secret RF eye bag protocol at Clifford Clinic is $2,500 for the first session and $1,800 for subsequent sessions. Many patients with mild eye bags need only one session. The doctor will advise you after assessing the size of your fat pads and your tissue response.

What results look like, by severity

Reviewing cases by grade matters more than reviewing a clinic’s single best photo, because the result you can realistically expect depends on your starting point.
- Grade 1 cases typically show subtle flattening of an early bulge
- Grade 2 cases show the clearest transformations from the protocol. A defined bag softening into a smooth lid-cheek transition over 1 to 2 months.

For both men and women, the protocol reduces existing fat rather than adding anything. This results the under-eye simply looks rested, with no change to facial shape or features.
Safety considerations
AGNES and Secret RF eye bag treatment is performed millimetres from the eye and should only be done by an experienced doctor with a thorough understanding of lower eyelid anatomy. At Clifford Clinic:

Frequently asked questions
What is the difference between AGNES and Secret RF? AGNES uses micro-insulated needles to deliver RF at a fixed depth into specific fat compartments. Together with Secret RF which is an insulated cannula that is inserted deep into the eye bag fat to tighten and remodel the overlying skin.
Can I just do AGNES without Secret RF? In selected cases with excellent skin quality, the doctor may adjust the protocol. However, for most patients, treating fat without supporting the skin risks a less smooth result. The recommendation is made at assessment.
Is the treatment safe so close to the eye? The insulated-needle design of AGNES confines energy to the target depth, and treatments are performed by experienced doctors with appropriate protective measures. Temporary side effects such as bruising and dry eyes can occur and are discussed before treatment.
How many sessions are needed? Often one. Some patients benefit from a second session ($1,800) depending on fat pad size and response, assessed at your review.
How long do results last? Typically 3 to 5 years. The treated fat does not regenerate in the same way, but ageing continues and skin laxity can develop, which is why some patients eventually consider surgery.
What if I’m told I’m not suitable? That happens regularly, and it is a feature of our approach, not a flaw. If your eye bags are Grade 3–4 or your skin is loose, we will recommend scarless eye bag surgery or pinch blepharoplasty instead. Both options available within Clifford Clinic.
Is AGNES + Secret RF suitable for men? Yes. Because the protocol reduces existing fat rather than adding volume, results in male patients look naturally rested rather than altered. Suitability criteria are the same. Mild Grade 1–2 bags with firm skin.
Can the protocol be combined with other treatments? Yes, where appropriate. Some patients have a residual tear trough or pigmentation addressed after the fat has settled at 1 to 2 months.
