Motiva is a breast implant brand, manufactured by Establishment Labs, widely used in Singapore and known for its SmoothSilk surface, ProgressiveGel fill options and built-in traceability technology. Its current range spans Round Plus, Ergonomix and Ergonomix2 implants, the JOY programme built around Ergonomix2, and Preservé, which is a minimal-dissection surgical approach rather than a separate implant. This guide explains what each name refers to, the two options available at The Clifford Clinic, namely Ergonomix (S$15,000 all-in) and the Premium JOY programme with Ergonomix2 (S$18,000 all-in), and what implant technology does not change. The Motiva Implant Brand

Motiva implants are round cohesive silicone gel implants distinguished by a portfolio of engineering features. The shell carries a fine SmoothSilk, also called SilkSurface, surface texture. The gel is supplied in ProgressiveGel formulations of differing viscoelasticity, meaning they differ in how readily the gel deforms and recovers its shape. TrueMonobloc construction bonds shell and gel into one structural unit, and a BluSeal barrier layer controls gel diffusion through the shell. An optional Q Inside, abbreviated Qid, passive radio-frequency identification microtransponder allows implant details to be verified after surgery with a handheld reader.

 

The Motiva Product Names Explained

• Motiva Round Plusis the more traditional round implant in the range, with balanced firmness and a fuller, rounder upper pole. It holds its shape rather than redistributing with posture, which suits patients who want definite upper-pole fullness.
• Motiva Ergonomixis built with a gel designed to move with posture, sitting rounder when the patient is lying down and more teardrop-shaped when upright. The intention is a natural appearance in motion without the rotation risk carried by fixed anatomical implants. “In most primary cosmetic augmentations, I can achieve a natural slope using a modern ergonomic implant without requiring a fixed anatomical shape,” says Dr Naidu. “I choose the implant according to the patient’s anatomy and desired result rather than a simple label like ‘round’ or ‘teardrop’.”
• Motiva Ergonomix2is the newer generation, engineered for greater softness and compressibility. Because the implant compresses more readily, it can be inserted through a smaller incision.
• JOY by Motivais not a different implant. JOY is a programme built around the Ergonomix2 implant, combining it with a defined surgical experience. At Clifford, the JOY programme includes Motiva’s extended warranty tier, with registration completed by the clinic. A patient choosing JOY therefore receives the Ergonomix2 implant plus the programme inclusions, not a different device.
• Preservéis a minimal-tissue-dissection surgical approach using dedicated Motiva implants and instrumentation, designed around smaller pockets and preservation of the surrounding tissue. Preservé is not currently part of Clifford’s line-up, which centres on Ergonomix and Ergonomix2.

Which Motiva implants are available at The Clifford Clinic?

The Clifford Clinic offers two Motiva packages, both performed by Dr Shenthilkumar Naidu, the first plastic surgeon in Singapore to use Motiva implants in 2016, one of the first surgeons handpicked by the manufacturer to work with its implants, and a Motiva-certified surgeon. Both are performed at The Clifford Surgery, an MOH-accredited day surgery centre.

• Motiva Ergonomix (‘Ergo 1’), S$15,000 all-in, before GST.The established gel implant designed to move with posture, and the option with the longest clinical track record in the Motiva range.
• Premium Motiva JOY with Ergonomix2 (‘Ergo 2’), S$18,000 all-in, before GST.The newer-generation implant with greater softness and compressibility, delivered within the JOY programme, which adds the extended warranty tier and clinic-completed registration.

At Clifford, patients can choose Qid-enabled or Qid-free implants, and every patient receives an implant card recording the serial numbers. Warranty registration is completed by the clinic on every patient’s behalf. Every Motiva implant carries the lifetime Always Confident Warranty® against rupture and a 10-year replacement policy for significant capsular contracture at Baker grade III to IV. The Premium JOY programme adds Motiva’s extended warranty tier, which contributes up to US$2,500 toward revision surgery costs for qualifying events, provided registration is completed within 90 days of surgery. The Manufacturer Claims and What They Do Not Prove

Motiva’s engineering features are real design differences, and the published outcome data are encouraging. A meta-analysis of 4,784 patients with SmoothSilk-surface Motiva implants reported an overall complication rate of 5.2% and capsular contracture of 0.54%, with no BIA-ALCL cases in the dataset (Aitzetmüller-Klietz et al., Journal of Clinical Medicine, 2023). A single-centre series of 5,813 consecutive augmentations reported an overall complication rate of 0.76% (Sforza et al., Aesthetic Surgery Journal, 2018). A six-year evaluation of 1,053 patients receiving Round and Ergonomix implants found low device-related complication rates (Randquist et al., Aesthetic Surgery Journal, 2023). Early comparative data on Ergonomix2 also report enhanced softness on quantitative elastography and improved contour-symmetry scores against the first generation. These figures describe cohorts rather than individuals, and manufacturer claims describe design intent rather than a guarantee of individual outcome. No surface, gel or shell technology eliminates capsular contracture, rupture, malposition or infection, and no implant is a lifetime device, which is the position of the United States Food and Drug Administration on all breast implants. A surgeon recommending Motiva should be able to explain which characteristics matter clinically for a given anatomy, and which are primarily marketing differentiation.

 

Motiva Implant Technology in Plain Language

Manufacturer terminology is dense, and the trade names below describe engineering choices rather than clinical outcomes. Each is set out in terms of what it means for a patient.

• SmoothSilk, also called SilkSurface.A fine, controlled surface produced without foreign particles, designed to reduce the inflammatory response and biofilm formation at the implant surface. Both are implicated in capsular contracture, the process in which scar tissue around the implant thickens and tightens. Under current surface classification standards it sits in the smooth category, which is relevant to the BIA-ALCL discussion below.
• ProgressiveGel, supplied as PLUS and ULTIMA. The gel families that determine how a Motiva implant behaves. ULTIMA is the highly viscoelastic gel used in Ergonomix, and it redistributes with gravity, which produces the round-when-lying and teardrop-when-upright movement. PLUS is more form-stable and holds a fuller round shape.
• Shell and gel engineered to behave as one unit under stress. Because the implant deforms as a single body rather than as a shell containing a separate filler, it can be inserted through a smaller incision without compromising the shell.
• A visible blue-tinted barrier layer confirming an intact gel-diffusion barrier on every implant, which the surgeon can verify by eye at insertion. The barrier limits silicone molecules from migrating slowly through the shell into the surrounding tissue, a process termed gel bleed.
• Q Inside, abbreviated Qid. An optional passive radio-frequency identification microtransponder inside the implant. A handheld reader can retrieve the implant’s identification without surgery, which is useful years later if records are lost. Patients who prefer no radio-frequency device can choose implants without it.

Each feature addresses a known implant problem, namely capsular contracture, rupture visibility, traceability and incision size. Each reduces the frequency of that problem rather than removing it, which is why the risk discussion below still applies in full and why long-term monitoring remains necessary whichever implant is chosen.

 

Choosing Between Ergonomix and the JOY Programme with Ergonomix2

Both implants are round and smooth-surfaced, and both are designed to move with posture. The practical differences are generation and feel. Ergonomix is the established option with the longest track record in the range, and its ULTIMA gel gives the movement and softness that suit patients prioritising a natural look in motion. Ergonomix2, delivered at Clifford within the Premium JOY programme, is engineered for greater softness and compressibility, so it can pass through a smaller incision and suits patients who rank softness highest. Patients with very thin tissue coverage should discuss both options against the placement plane, because the softer gel of Ergonomix2 conforms more readily to the overlying tissue and can therefore show implant edges where coverage is limited. The right choice follows the same logic as any implant selection, which is measurements first, priorities second and model last. Dr Naidu describes his own criteria as follows. “I do not choose an implant simply because it is newer or marketed as more advanced. The decision starts with the patient’s chest width, existing breast tissue, skin quality, degree of asymmetry, desired volume and the type of result she wants. In practice, I would be more inclined towards Ergonomix2 when the patient places a high priority on softness, natural movement and minimising the incision length, provided that the implant dimensions suit her anatomy. Ergonomix remains a reasonable choice for a patient who wants a natural result but prefers an implant with a longer clinical track record. The implant generation is only one part of the decision. Implant width, projection, volume and the quality of the tissue covering the implant may have an even greater influence on the final result.”

 

Motiva Implants and BIA-ALCL Risk

Breast implant-associated anaplastic large cell lymphoma, abbreviated BIA-ALCL, is a rare lymphoma of the scar capsule rather than a breast cancer, and it has been far more strongly associated with textured implants than with smooth ones. Motiva’s SmoothSilk surface is classified as a smooth surface, which places it in the lower-risk category on current evidence. A rare risk cannot be excluded for any implant, and the standard advice applies regardless of brand. Late swelling, pain or a mass around any implant warrants prompt assessment rather than observation, because the condition is highly treatable when the capsule and implant are removed early. Motiva Breast Implant Cost in Singapore

At The Clifford Clinic, Motiva breast augmentation is an all-inclusive S$15,000 with Ergonomix, or S$18,000 for the Premium JOY programme with Ergonomix2, both before GST. The S$3,000 difference reflects the implant generation and the programme inclusions, which are the extended warranty tier and the defined JOY surgical pathway, rather than a difference in surgeon or facility. An all-in quotation matters more than the implant price alone, because a figure quoted for the implant by itself excludes the surgeon and anaesthetist fees, the day-surgery facility charge, compression garments and post-operative reviews, and those components determine what a patient actually pays. Motiva Warranty and Traceability in Practice

Two administrative details at the time of surgery determine what protection remains available decades later. The first is registration. Motiva’s warranty programmes require the implants to be registered after surgery, and Clifford performs this registration on the patient’s behalf. The Premium JOY programme includes the extended warranty tier, which adds the Motiva Extended Warranty to the lifetime Always Confident Warranty®, contributing up to US$2,500 toward revision surgery for a qualifying rupture or capsular contracture and activated by registration within 90 days of surgery. An unregistered implant can mean an unclaimable warranty, so patients should confirm that registration has been completed before leaving the first post-operative review. The second is documentation. Every patient should hold an implant card recording model, size and serial numbers, kept independently of any clinic’s records. Qid-equipped implants add a further safety net, because identification can be read directly from the device, but the card remains the primary document for imaging referrals, warranty claims and any future safety notice. It should be retained permanently and shown to any doctor who takes over the patient’s care.

 

How Suitability for Motiva Implants Is Assessed

Suitability is assessed the same way as for any implant. The assessment covers measurements of breast-base width and tissue thickness, assessment of the skin envelope and symmetry, discussion of goals and lifestyle, and matching of implant diameter, projection and gel to that anatomy, described in detail in how implant size and profile are selected. Motiva’s model range changes which combinations of diameter, projection and gel are available. It does not change the planning logic.

The meaningful comparison is which system’s configurations fit a patient’s anatomy and priorities.

What is the Preservé technique?

A Motiva-designed surgical approach using minimal tissue dissection, dedicated instrumentation and specific implants to create a smaller, more precise pocket. Preservé is not currently offered at Clifford, whose Motiva programmes centre on Ergonomix and Ergonomix2 within the JOY programme.

Can the Qid chip be detected by airport scanners or affect MRI?

Qid is a passive device with no battery, so it does not transmit and is not detected by airport security scanners. It is MR Conditional, meaning MRI can be performed under defined conditions, and the transponder can create a small localised artefact on breast MRI images near the device. Patients should mention it when booking any breast imaging.

Is Ergonomix2 worth the higher price?

That depends on what the patient is paying for. The newer generation offers greater softness and a smaller incision, and the JOY programme adds defined inclusions. For some patients Ergonomix meets every goal at lower cost. This is a trade-off to work through at consultation rather than to decide from a price list.
