Eye bags are one of the most common and most misunderstood aesthetic concerns we see at The Clifford Clinic. Patients often ask why they have eye bags despite sleeping well and feeling rested. Eye creams, cold compresses and more sleep also seem to do nothing to fix the problem. The reason for this is because eye bags are actually not a problem caused by tiredness, they are an anatomical structural problem.

Eye bags are among the most common aesthetic concerns encountered at Clifford Clinic, and among the most frequently misdiagnosed. The presenting history is consistent. Patients report adequate sleep and no subjective fatigue, yet are repeatedly told by colleagues that they look tired. Eye creams, cold compresses and additional sleep have usually been tried without effect. The cause is anatomical rather than a matter of fatigue.

In appropriately selected patients, eye bags can be treated effectively without surgery. Patient selection is the determining variable. Non-surgical eye bag removal is not a single universal treatment, and not every energy device or injectable marketed for the under-eye area acts on the protruding fat that produces an eye bag.

What causes eye bags?

True eye bags are caused by lower eyelid fat protrusion. Three small fat pads sit within the orbit (the eye socket), cushioning the eyeball. They are normally held back by the orbital septum, a thin, fibrous membrane. With age, or earlier where there is a familial predisposition, this membrane weakens and the fat pushes forward. The result is the soft, persistent bulge below the lash line termed an eye bag.

Two consequences follow from this anatomy.

1. 1. An eye bag is a structural problem.The fat has physically moved forward. It does not respond to sleep, hydration, creams or massage, because none of these measures can reposition a herniated fat pad behind a weakened membrane.
   2. An effective treatment must act on the fat itself, either reducing its volume or removing and repositioning it surgically. Treatments that only warm, tighten or “rejuvenate” the overlying skin leave the bulge unchanged, which is the principal limitation of several treatments marketed for the indication.

Eye bags vs tear troughs and dark eye circles

Not every under-eye concern is a true eye bag, and the wrong self-diagnosis leads directly to the wrong treatment. The conditions most commonly confused with one another are set out below.

We recommend always consulting a doctor but if you would like to a simple home check, gently stretch the under-eye skin in a mirror. Darkness that persists despite stretching the skin indicates dark eye circles.  Darkness that disappears when the skin is stretched are likely shadows casted by either a tear trough or an eye bag bulge.

 

A simple assessment can be performed at home by gently stretching the under-eye skin in front of a mirror. Darkness that persists is pigment. Darkness that resolves is a shadow cast by either a groove (trough) or a bulge (bag). Puffiness that fluctuates with salt intake, sleep or allergies is fluid rather than fat. How eye bags are graded and why the grade decides the treatment

Eye bags are commonly graded from 1 to 4.

• Grade 1.Early, mild fat protrusion, visible mainly in certain light or on upward gaze.
• Grade 2.A defined, constant bulge, with the lid-cheek transition preserved.
• Grade 3.Significant protrusion, often with a deepening groove beneath the bag.
• Grade 4.Severe protrusion, usually with loose skin and visible distortion of the lid-cheek junction.

The grade, together with skin quality, is the principal determinant in the choice between non-surgical and surgical treatment.

Who is suitable for non-surgical eye bag removal?

Non-surgical eye bag removal with AGNES and Secret RF is most suitable for patients who have both of the following.

• Mild Grade 1 to 2 eye bags, representing early to moderate fat protrusion.
• Reasonably firm lower eyelid skin.

Why lower eyelid skin quality determines suitability

After any fat reduction, the overlying skin must contract and re-drape over the flattened contour. Firm, elastic skin retracts readily. Lax, crepey skin does not, and may leave the area appearing deflated or wrinkled rather than refreshed.

A good non-surgical result requires that the periorbital skin is not lax. Older patients more often require surgery, since excess skin must be excised with a pinch blepharoplasty in addition to the fat. The criterion is anatomical rather than chronological, although skin laxity accumulates with age, so the two correlate closely.

Patients with Grade 3 to 4 eye bags, significant loose skin or lower eyelid laxity are advised towards scarless eye bag surgery instead.

 

How AGNES + Secret RF eye bag removal works

Clifford Clinic treats eye bags with a two-step protocol, refined over more than a decade.

Step 1: AGNES for precision fat reduction

AGNES is a radiofrequency (RF) device that delivers energy through fine micro-insulated needles. The insulation along the needle shaft means RF energy is released only at the exposed tip, at a fixed, controlled depth. This allows the doctor to treat the specific fat compartments responsible for the bulge, whether one, two or all three of the lower fat pads, while sparing the overlying skin and surrounding structures. The treated fat is coagulated, then broken down and resorbed by the body over the following weeks, and the bulge flattens as it clears.

Step 2: Secret RF for deeper fat reduction

Secret RF uses a single direct insulated cannula with a radiofrequency tip. The cannula performs accurate deeper ablation of the eye bag fat at a different depth from AGNES, while the heat that travels to the overlying skin drives new collagen formation over the following weeks, allowing the skin to contract smoothly over the reduced fat pad rather than draping loosely across it.

 

Why two steps instead of one device

A true eye bag involves two layers, the protruding fat and the skin envelope above it. Treating fat without supporting the skin risks a deflated contour. Tightening skin without reducing fat leaves the bulge untouched. Dividing the work between two devices allows each to be used at conservative settings for the single indication it addresses best, which matters when treating within millimetres of the eye. It also permits more selective targeting of the individual fat compartments than a single device applied across the whole area.

 

An honest review of other treatments marketed for reducing eye bags

Our clinical assessment of each of the treatments listed below is based on the treatment’s ability to affect the protruding fat, which is the underlying anatomical cause of the eye bag. 

 

Other treatments marketed for eye bags, assessed

Patients regularly present having researched, or already tried, other options. Each is assessed below against the anatomical test set out above, namely whether the treatment acts on the protruding fat.

Tear trough fillers

Fillers are an appropriate treatment for a genuine tear trough or mid-cheek deficiency, both of which are volume deficits. They are not an eye bag treatment. Filler cannot reduce protruding fat, and adding volume around a bulge usually makes the under-eye look heavier. When the patient smiles, the filler can bunch into a visible lump. Where a patient has both a bag and a trough, the fat is treated first and the trough reassessed after 1 to 2 months, by which point it has frequently resolved along with the bulge.

HIFU (high-intensity focused ultrasound)

HIFU has a legitimate role in skin tightening elsewhere on the face, where it delivers focused heat at fixed focal depths of approximately 1.5, 3.0 and 4.5 mm. Around the eyes the same depth profile becomes a liability. Lower eyelid skin is among the thinnest on the body, at roughly half a millimetre, so the tissue of interest sits well above the plane at which the focal energy is deposited. More fundamentally, HIFU does not reduce the volume of the protruding fat, nor does it reposition it behind the septum. HIFU is therefore not a useful treatment for true eye bags.

Fotona “4D” / “5D” / “6D” laser protocols

Several clinics market multi-step Fotona laser protocols that include heating the lower eyelid from within by directing a long-pulse neodymium YAG laser at the conjunctiva. The rationale is difficult to sustain. Heating tissue, from either side, neither removes orbital fat nor reduces the volume of the fat pad responsible for the bulge. The wavelength is the relevant point. Long-pulse neodymium YAG light is absorbed principally by water and therefore heats tissue non-selectively, unlike the 1444 nm wavelength used in laser lipolysis, which is preferentially absorbed by fat. Skin heating may produce mild, temporary tightening of the surface, but the bulge beneath is unchanged. No step in these protocols removes or reduces orbital fat.

Rejuran for eye bags or dark circles

Rejuran (polynucleotide injections) is popular in Singapore for general skin conditioning. For the under-eye concerns discussed here, however, it has no meaningful role. It does not thicken the skin, it is not a collagen biostimulator, it does not lighten dark circles, and it does not reduce protruding fat. Patients are frequently surprised by this, since Rejuran is so widely offered for the under-eye area.

Re20 Fine and Juvelook Eyes, useful adjuncts for skin quality

Re20 Fine and Juvelook Eyes are collagen-stimulating injectables that are useful around the under-eyes for improving skin quality. Their role is the skin envelope rather than the fat. They can complement an eye bag treatment plan or address fine crepiness in suitable patients, but indepently, they will not flatten a protruding fat pad.  Worthwhile when used for the appropriate indication, and ineffective when sold as eye bag removal.

Eye creams and home remedies

Cold compresses, sleep, salt reduction and caffeine creams do relieve fluid puffiness, although the effect is temporary. They have no effect on structural fat protrusion. Where the appearance is identical every morning, the underlying problem is fat rather than fluid, and topical measures are unlikely to alter it.

Downtime and side effects

Non-surgical eye bag removal is not a no-downtime procedure. The typical course of recovery is set out below.

• About 4 to 5 days of mild bruising and swellingis typical. Most patients return to work within a few days, using concealer for any residual bruising.
• Temporary dry eyes for 1 to 2 daysoccur in some patients, and respond to lubricating drops.
• A small, palpable firmnesscan occasionally be felt under the eye as the treated fat breaks down and temporarily coalesces before resorption is complete. It is usually not visible externally and typically settles without intervention. It is one of the findings the review appointment exists to monitor.

Cold compresses, an elevated sleeping position, and avoidance of alcohol, strenuous exercise and saunas for about 5 to 7 days all shorten the visible recovery. Contact the clinic promptly for any vision changes, worsening pain or redness, or a lump that is enlarging, painful or externally visible. These are uncommon, but the threshold for review should be low for treatment adjacent to the eye.

 

Results and how long they last

Early improvement is often visible from around 2 weeks as swelling settles, with full results over 1 to 2 months as the treated fat clears and collagen remodelling completes. Because the fat is reduced rather than concealed, results are durable, typically 3 to 5 years. Ageing nevertheless continues. The same septal weakening that produced the first bag can gradually produce new protrusion, and skin laxity develops over time, so some patients transition to surgery years later. Individual results vary.

• Previous non-surgical treatment gave incomplete results because the underlying anatomy was beyond its scope.

Is non-surgical eye bag removal painful?

Topical numbing is applied for about 30 minutes and local anaesthesia may be used. Most patients report the 20-minute procedure to be well tolerated.

How many sessions will I need?

Is Rejuran worth trying for my dark circles or bags first?

Rejuran does not thicken skin, stimulate collagen meaningfully, lighten pigmentation or reduce fat, and is therefore not a useful first step. Collagen stimulators such as Re20 Fine or Juvelook Eyes are more useful for under-eye skin quality, but they are still not eye bag treatments.

What if my skin is loose?

Loose lower eyelid skin makes non-surgical treatment unsuitable. Surgery, often with a pinch blepharoplasty, gives a more complete result.

Am I a candidate if my problem is dark circles?

Dark circles are a different condition requiring different treatment. Many dark circles are associated with allergic rhinitis. In those cases the more accurate approach is to treat the underlying allergy first, beginning with an allergy blood test and a skin prick test followed by sublingual immunotherapy (SLIT), while simultaneously treating the dark circles themselves with V Beam Eyes and Juvelook Eyes. Treating the colour without addressing the rhinitis usually means the circles return. Examination at consultation establishes which condition is present.
