Eye bags are one of the most common and most misunderstood aesthetic concerns we see at The Clifford Clinic. Patients often ask why they have eye bags despite sleeping well and feeling rested. Eye creams, cold compresses and more sleep also seem to do nothing to fix the problem. The reason for this is that eye bags are actually not a problem caused by tiredness, they are an anatomical structural problem.

Eye bags, however, are not untreatable. Mild eye bags can even be treated effectively without surgery. This guide aims to be a complete review of non-surgical eye bag removal in Singapore, written from the experience of our doctors who have performed AGNES and Secret RF eye bag treatments for over 10 years and treated over 1000 patients.

What causes eye bags?

True eye bags are caused by lower eyelid fat protrusion. Three small fat pads sit within the eye socket to cushion the eyeball. While they are normally held back by a thin, fibrous membrane known as the orbital septum, age weakens this membrane and allows the fat to push forward. The result is a soft, persistent bulge below the lash line that we call an eye bag. Eye bags can also form earlier in life if you have a genetic predisposition for developing them.

Below are the two most important things to note when dealing with eye bags.

  1. An eye bag is a structural problem. It is caused by fat physically moving forward. Eye bags therefore do not respond to sleep, hydration, creams or massages because none of these treatment methods can reposition a herniated fat pad.
  2. The treatment must target the fat to be effective. Eye bags cannot be eliminated unless the problematic fat is reduced, removed or repositioned. Treatments that only warm, tighten or rejuvenate the overlying skin will not have an effect as they do not deal with the root cause of the eye bag.

Eye bags vs tear troughs and dark eye circles

Not every under-eye concern is a true eye bag. It is important to get a professional diagnosis as an incorrect self-diagnosis can lead to choosing a treatment that does not fix the underlying cause of the problem.

We recommend always consulting a doctor but if you would like to a simple home check, gently stretch the under-eye skin in a mirror. Darkness that persists despite stretching the skin indicates dark eye circles.  Darkness that disappears when the skin is stretched are likely shadows casted by either a tear trough or an eye bag bulge.

 

How eye bags are graded and why the grade decides your treatment

Doctors commonly grade eye bags from 1 to 4:

  • Grade 1: Mild fat protrusion that is visible under certain lighting or when looking up.
  • Grade 2: A defined, constant bulge under the eye.
  • Grade 3: Significant protrusion of the under-eye bulge. Often with a deepening groove beneath the bag.
  • Grade 4: Severe protrusion of the under-eye bulge. Accompanied by loose skin and visible distortion of the lid-cheek junction.

The eye bag grade and overall skin quality are the most important factors that determine if the patient should choose non-surgical or surgical treatment.

Who is suitable for non-surgical eye bag removal?

Non-surgical eye bag removal with AGNES + Secret RF is recommended for patients with mild Grade 1 to 2 eye bags, early to moderate fat protrusion, and reasonably firm lower eyelid skin.

 

Why skin laxity matters

The skin quality, while often glossed over, affects the overall results achieved. After any fat reduction, the overlying skin must contract and re-drape over the flattened contour. While firm and elastic skin does this well, loose skin does not. This can result in the area looking deflated or wrinkled rather than refreshed.

In our experience,  good skin elasticity around the eyes is needed for non-surgical treatment to give good results. This is why older patients generally require surgery to get good results. Beyond just removing the fat, they often need the excess skin to be removed via pinch blepharoplasty.

Patients with Grade 3–4 eye bags, significant loose skin or lower eyelid laxity are advised towards scarless eye bag surgery instead. Because Clifford Clinic offers both pathways, our recommendation follows your anatomy, not the limits of our equipment.

 

How AGNES + Secret RF eye bag removal works

The Clifford Clinic treats eye bags with a two-step protocol.

 

Step 1: AGNES for precision fat reduction

AGNES is a precision radiofrequency device that uses micro-insulated needles to target fat. The insulation along the needle shaft means RF energy is released at a controlled depth only at the exposed tip. This allows the doctor to treat the specific fat compartments responsible for the eye bag bulge while sparing the overlying skin and surrounding structures. The treated fat breaks down gradually and is cleared by the body over the following weeks, flattening the eye bag.

Step 2: Secret RF for deeper fat reduction

Secret RF has a single direct insulated cannula that has RF energy. It allows for accurate deep ablation of the fat. Accurate placement and controlled delivery of this RF allows for direct fat ablation of the eye bag fat. Moreover, the heat supplied also travels to the top of the skin, ensuring that the skin contracts smoothly over the reduced fat pad.

 

Benefits of a combination protocol  

A true eye bag involves two layers, the protruding fat and the skin envelope above it. Treating fat without supporting the skin risks creating a deflated look, while tightening the skin without reducing fat leaves the bulge untouched. Using a single device may result in addressing only one issue while ignoring the other as most devices are fine-tuned to target a specific skin concern. An honest review of other treatments marketed for reducing eye bags

Our clinical assessment of each of the treatments listed below is based on the treatment’s ability to affect the protruding fat, which is the underlying anatomical cause of the eye bag.

 

Tear trough fillers

Fillers are an appropriate treatment for a tear trough or mid-cheek deficiency as both are caused by a volume deficit. However, they are not suitable for treating eye bags.

Fillers cannot reduce protruding fat and adding volume around the fat bulge can make the under-eye look heavier. The filler may also bunch into a visible lump when the patient smiles. Where a patient has both an eye bag and a trough, we recommend treating the fat first then reassess the trough after 1 to 2 months as the trough frequently disappears along with the fat bulge.

 

HIFU (high-intensity focused ultrasound)

While HIFU delivers focused heat at depths of several millimetres to tighten skin. While it is effective for skin tightening elsewhere on the face, its treatment depth becomes problematic when dealing with the skin around the eyes.

The lower eyelid skin is among the thinnest on the bod. As HIFU’s focal energy is designed for deeper planes, it is not suited for treating the eye area. More importantly, HIFU cannot melt the protruding fat nor can it push the fat back behind the septum. In our assessment, HIFU is thus not a useful treatment for eye bags.

 

Fotona 4D / 5D / 6D laser protocols

Fotona laser heats the lower eyelid from the inside by directing laser energy at the conjunctiva to produce mild, temporary tightening of the skin surface. While marketed by some clinics as an eye bag removal treatment, we find that it is not effective for treating eye bags as heating tissue does not remove orbital fat or shrink the fat pad that causes the bag.

 

Rejuran for eye bags or dark circles

Rejuran is a polynucleotide injection that is popular in Singapore for general skin conditioning. For the purposes of eye bag removal, however, our assessment is that it has no meaningful impact. While it can improve skin elasticity, it ultimately cannot lighten dark eye circles or reduce protruding fat.

 

Re20 Fine and Juvelook Eyes — useful adjuncts, for the right job

Re20 Fine and Juvelook Eyes are collagen-stimulating treatments that we feel are useful adjuncts for eye bag treatment. Re20 Fine and Juvelook Eyes improve skin quality through collagen stimulation. While they will not flatten a protruding fat pad on their own as they target the skin envelope and not the fat directly, they can complement an eye bag treatment plan or address fine crepiness in suitable patients.

Eye creams and home remedies

Cold compresses, sleep, salt reduction and caffeine creams can reduce fluid puffiness temporarily but they do nothing to adress structural fat protrusion.

Downtime and side effects

The Clifford Clinic does not advertise AGNES + Secret RF eye bag removal as a zero downtime treatment as in our experience treating over 1000 patients, mild bruising and swelling is usually expected post treatment.

  • About 4 to 5 days of mild bruising and swellingis typical. Most patients return to work within a few days. Concealer can be applied to mask bruising.
  • Temporary dry eyes for 1 to 2 daysoccur in some patients. Lubricating drops can be applied to alleviate dryness.
  • A small, palpable firmnesscan occasionally be felt under the eye as the treated fat breaks down and temporarily clumps before the body clears it. Such lumps are usually not visible externally and typically settle on their own.

Cold compresses, sleeping slightly elevated, and avoiding alcohol, strenuous exercise and saunas for about 5 to 7 days post treatment. Contact the clinic promptly if you experience vision changes, worsening pain, redness, or if you develop a lump that is painful or visible.

 

What results to expect and how long they last

Early improvement is often visible from around 2 weeks after the treatment as swelling settles. Full results typically take over 1 to 2 months to show as the treated fat clears and collagen remodelling completes. Because the fat is reduced rather than masked, results are long-lasting, typically lasting 3 to 5 years.

Do note that as ageing continues, the same septal weakening that produced the first bag can gradually produce a new protrusion. Due to skin laxity also developing or worsening over time, some patients eventually choose to do an eye bag removal surgery years later.

Most patients with mild Grade 1–2 eye bags need only one or two sessions. When surgery is the better choice

Non-surgical treatment may not be the best choice for all patients. Below are some examples of when surgery is generally more appropriate.

  • Eye bags that are Grade 3–4.
  • There is significant loose or excess lower eyelid skin that requires a pinch blepharoplasty to fix.
  • Previous non-surgical treatment gave incomplete results due to the severity of the problem.

For these patients, surgery is usually both the better and the more economical option.

Frequently asked questions

Is non-surgical eye bag removal painful? Topical numbing is applied about 30 minutes before the treatment. Local anaesthesia may be used if needed. Most patients describe the 20-minute procedure as very tolerable.

How many sessions will I need? Typically one session for mild Grade 1–2 eye bags. Some patients with more severe eye bags may benefit from a second session.

Is Rejuran worth trying for my dark circles or bags first? In our assessment, no as it does not thicken skin, lighten pigmentation or reduce fat. What if my skin is loose? Loose lower eyelid skin makes non-surgical treatment unsuitable. Surgery, often with a pinch blepharoplasty, will give a more complete result.

Am I a candidate if my problem is actually dark circles? Dark circles are a different condition requiring different treatment. Dark circles are also associated with allergic rhinitis. For such cases, we recommend treating the underlying allergy first as treating the pigmentation without addressing the rhinitis usually means the dark circles will return.
