Non-Surgical Eye Bag Removal with AGNES & Secret RF

Eye bags are a common concern for people who notice puffiness, bulging, or heaviness under the eyes.

While lack of sleep can make the under-eye area look more swollen temporarily, persistent eye bags are often linked to bigger structural changes, such as under-eye fat protrusion, skin laxity, fluid retention, genetics, and ageing-related weakening of the tissues around the eyes.

At The Clifford Clinic, non-surgical eye bag removal is performed using a doctor-assessed combination of AGNES RF and Secret RF. This approach uses radiofrequency energy delivered through fine microneedles to target selected under-eye tissue without surgical incisions.

This treatment may be suitable for patients with mild to moderate eye bags who prefer a non-surgical option, but it is not the right treatment for every under-eye concern.

Book a doctor consultation to find out whether AGNES + Secret RF is suitable for your under-eye concern.

What causes eye bags?

Eye bags form when the area beneath the lower eyelids becomes puffy, swollen, or visibly bulging. In many cases, this happens because the supporting structures around the eyes weaken over time. Fat that normally supports the eye may shift forward into the lower eyelid area, while the skin and surrounding tissues may become looser. Fluid can also collect under the eyes, making puffiness more noticeable.

Common contributing factors include:

  • Ageing and reduced skin firmness
  • Genetics or family tendency
  • Under-eye fat protrusion
  • Skin laxity around the lower eyelids
  • Fluid retention, especially after salty meals or upon waking
  • Poor sleep or irregular sleep patterns
  • Allergies or sinus-related swelling
  • Smoking and lifestyle factors
  • Certain medical conditions, including thyroid, kidney, skin, or inflammatory conditions

Not all under-eye puffiness is caused by true eye bags. Temporary swelling from allergies, fluid retention, irritation, or lack of sleep may improve when the underlying trigger is managed. Persistent or worsening swelling, especially if it is one-sided, painful, associated with redness, vision changes, rash, headaches, or other symptoms, should be assessed by a healthcare professional.

Eye bags vs dark circles vs tear trough hollows

Many people use the term “eye bags” to describe any under-eye concern, but eye bags, dark circles, and tear trough hollows are different problems. They can also appear together, which is why a proper assessment is important before deciding on treatment.

Under-Eye Concerns: Eye Bags, Dark Circles and Tear Trough Hollows
Concern What It Usually Looks Like Common Causes Suitable Treatment Approach
Eye Bags Puffy or bulging area under the lower eyelid Fat protrusion, skin laxity, fluid retention, ageing and genetics AGNES + Secret RF may be considered when under-eye fat and mild skin laxity are contributing factors
Dark Circles Brown, grey, blue or purple-looking darkness under the eyes Pigmentation, thin skin, visible blood vessels, dermatitis, rubbing, genetics and shadowing Treatment depends on the cause and may involve skincare, pigment treatments, vascular lasers or other options
Tear Trough Hollows A hollow groove between the lower eyelid and upper cheek Volume loss, facial anatomy, ageing and genetics Fillers or other volume-restoring treatments may be considered in suitable cases

Dark circles can result from multiple causes, including pigmentation, thin translucent skin, visible blood vessels, puffy eyelids, fat loss, tear trough hollowing, and shadowing from the shape of the orbit. Cleveland Clinic also notes that tear trough hollows can create shadows that make puffiness or darkness look more obvious.

This distinction matters because AGNES RF and Secret RF is mainly used to address selected cases of under-eye fat prominence and skin laxity. It is not a pigment-lightening treatment and may not be the most suitable option if the main issue is brown pigmentation, vascular darkness, or a hollow tear trough.

Who is suitable for non-surgical eye bag removal?

Non-surgical eye bag removal with AGNES + Secret RF may be suitable for patients who have:

  • Mild to moderate under-eye bags
  • Visible lower-eyelid puffiness caused partly by fat prominence
  • Mild to moderate skin laxity around the lower eyelid
  • A preference for treatment without surgical incisions

It may be less suitable for patients with:

  • Severe eye bags with significant excess skin
  • Marked lower eyelid sagging or looseness
  • Very prominent tear trough hollows
  • Dark circles caused mainly by pigmentation or visible blood vessels
  • Temporary puffiness from allergies, fluid retention, or irritation
  • Active infection, rash, wounds, or inflammation near the treatment area
  • Medical conditions that require assessment before aesthetic treatment

Overall, AGNES + Secret RF is generally more suitable for patients whose eye bags and skin laxity are not too severe, while larger eye bags may require multiple sessions or may need other treatment options.

There are 3 types of eye bags conditions:

TYPE I

  • Mild eyebag fat accumulation under the eyes
  • Not prominent
  • Visible only when looking up

TYPE II

  • Accumulation of eyebag fats in a semi-circle under the eyes
  • Prominent when looking straight ahead

TYPE III

  • Large amount of eyebag fat accumulation in a semi-circle under the eyes
  • Very prominent
  • Eyes look swollen and tired

How AGNES + Secret RF eye bag removal works

AGNES and Secret RF are radiofrequency-based treatments that use fine microneedles to deliver RF energy into targeted tissue layers. The goal is to address selected under-eye fat and support skin tightening through controlled heat energy.

Agnes RF

AGNES RF is used to deliver focused RF energy through insulated microneedles. Clifford’s existing content describes AGNES as delivering fixed-depth, concentrated RF energy at the microneedle tip.

secret rf

Secret RF is a fractional radiofrequency microneedling system designed to deliver controlled energy at different depths. Cutera describes Secret RF as a fractional RF microneedling system designed to stimulate and remodel collagen from the inside out.

When used together for non-surgical eye bag removal, the treatment is intended to address two common contributors to eye bags:

  1. Under-eye fat prominence
RF energy is delivered into selected tissue planes to help reduce the appearance of bulging under-eye fat.
  2. Skin laxity and texture around the lower eyelid
Controlled RF energy may support collagen remodeling and skin tightening over time.

Because the under-eye area is delicate, treatment parameters should be selected by a doctor based on the patient’s anatomy, skin thickness, severity of eye bags, and suitability for RF treatment.

Why Clifford uses a two-step RF approach

Eye bags are often not caused by only one factor. A patient may have under-eye fat bulging, skin laxity, mild hollowing, and pigmentation at the same time. Treating only one part of the problem may not give a balanced result.

The Clifford Clinic’s two-step RF approach combines AGNES and Secret RF to address different tissue levels and treatment goals. AGNES delivers fixed-depth, concentrated RF energy, while Secret RF delivers more controlled RF energy across different layers of the under-eye fat area.

This combined approach may be considered when both under-eye fat prominence and skin laxity are present. The exact treatment plan should be customised after assessment, as not every patient needs the same depth, intensity, or number of sessions.

Non-surgical eye bag removal vs fillers vs surgery

The best treatment depends on what is causing the under-eye concern. Non-surgical RF, fillers, and surgery address different problems.

Non-Surgical and Surgical Eye Bag Treatment Options
TreatmentBest Suited ForHow It WorksKey Limitations
AGNES + Secret RFMild to moderate eye bags caused by fat prominence and mild skin laxityUses RF microneedling to target selected under-eye tissue and support tighteningResults are gradual, may require multiple sessions, and may be limited for severe eye bags
Tear Trough FillersHollowness or shadowing under the eyesAdds volume to soften the hollow groove between the eyelid and cheekDoes not remove true eye bag fat and may not be suitable if puffiness is the main concern
Lower Eyelid Surgery / BlepharoplastyMore severe eye bags, significant fat protrusion or excess skinSurgically removes or repositions fat and may remove excess skinInvolves surgery, recovery time and surgical risks

For some patients, combination treatment may be more appropriate than one single procedure. For example, a patient with true eye bags and tear trough hollows may need different treatments for each component. A doctor consultation helps determine whether AGNES + Secret RF, fillers, surgery, lasers, or a combination approach is more suitable.

Most importantly, AGNES and Secret RF are safe and effective for use on the delicate eye area!

Treatment process

A typical AGNES + Secret RF treatment process may include:

1. Consultation and assessment
The doctor examines the under-eye area and checks the severity, skin quality, anatomy, and likely cause of the eye bags.

2. Treatment planning
The treatment area is planned according to the location of the under-eye bulge and the degree of skin laxity.

3. Numbing and local anaesthesia
Topical numbing cream and local anaesthesia may be used to improve comfort. The current Clifford page states that numbing cream and local anaesthesia are used before treatment.

4. AGNES RF treatment
AGNES is used to deliver focused RF energy into selected areas beneath the lower eyelid.

5. Secret RF treatment
Secret RF is then used to deliver controlled RF microneedling energy to support skin tightening and collagen remodeling.

6. Cooling and immediate aftercare
Cooling packs may be applied to soothe the treated area. Clifford’s existing page also describes cooling gel packs being placed on the eye area after treatment.

7. Review and follow-up
A follow-up review may be arranged to assess healing, swelling, and whether additional sessions are required.

Downtime, swelling and aftercare

AGNES + Secret RF does not involve surgical incisions, but the under-eye area may still swell or bruise after treatment.

Possible temporary effects include:

  • Mild swelling
  • Bruising
  • Redness
  • Tenderness
  • Small needle marks
  • Temporary unevenness during healing

Aftercare may include:

  • Using cold compresses as advised
  • Avoiding rubbing or massaging the under-eye area
  • Avoiding facials or forceful facial treatments until cleared
  • Keeping the area clean
  • Avoiding intense heat, sauna, and strenuous exercise for the period advised
  • Avoiding alcohol during the treatment period if advised
  • Checking with the doctor before stopping any medication or supplement that may affect the risk

Patients should not stop prescription medication on their own. If you are taking blood thinners, anti-inflammatory medication, supplements, or have a medical condition, inform the doctor before treatment so that advice can be tailored safely.

Results: when to expect improvement

Results from non-surgical eye bag removal are usually gradual. Some early changes may be noticed as swelling settles, while collagen remodeling and tissue changes can continue over the following weeks.

Patients may notice improvement in as little as two weeks, while maximum results may take about one to two months. AGNES + Secret RF may require one to three sessions over one to two months, depending on the individual case.

Our doctors will advise what kind of improvement is realistic after examining the under-eye area.

Risks and limitations

AGNES + Secret RF is a non-surgical treatment, but like all medical aesthetic procedures, it carries potential risks and limitations. Temporary side effects may include swelling, bruising, redness, tenderness, mild discomfort, and small needle marks around the under-eye area.

Less common risks may include temporary skin colour changes, uneven results, under-correction, infection, burns, scarring, or other complications. Results also vary between patients, and more than one session may be required depending on the severity of the eye bags and individual healing response.

AGNES RF and Secret RF may not fully correct severe eye bags, significant excess skin, pigmentation-related dark circles, or deep tear trough hollows. A doctor consultation is important to assess your under-eye anatomy, determine suitability, and explain what level of improvement is realistic.

Why choose The Clifford Clinic?

At The Clifford Clinic, non-surgical eye bag removal begins with a detailed doctor assessment. This is important because under-eye concerns can look similar, but may have very different causes. What appears to be an eye bag may actually be caused by tear trough hollows, pigmentation, vascular dark circles, fluid retention, skin laxity, or a combination of factors.

Our doctors assess your under-eye anatomy carefully, including the severity of fat protrusion, degree of lower-eyelid skin laxity, skin thickness, sensitivity, and the presence of dark circles or hollowness. This allows us to determine whether AGNES + Secret RF is suitable for your condition, or whether another treatment such as fillers, lasers, surgery, or a combination approach may be more appropriate.

The Clifford Clinic uses a two-step RF approach with AGNES and Secret RF to address selected cases of mild to moderate eye bags without surgical incisions. Treatment is planned according to your individual anatomy, rather than using a one-size-fits-all method.

During consultation, our doctors will also explain the expected downtime, potential risks, number of sessions required, likely results, and treatment cost. The goal is to help you understand what can realistically be achieved, so you can make an informed decision with confidence.

FAQ

AGNES and Secret RF eye bag removal treatment is suitable for patients who aren’t suffering from severe eye bags. The treatment works best in patients where skin laxity and eye bag size is not too big; however, with the addition of Secret RF – it is possible to reduce the size of larger eye bags, although multiple sessions may be required.

Topical numbing cream and injection of local anaesthesia will be administered before the treatment. However, you may still feel a warm and slightly prickly sensation during the treatment.

The total treatment time takes about 20 minutes, while the numbing procedure takes about 30 minutes.

It takes roughly 1 or 2 months to see maximum results. However, some patients notice improvements in as little as the first 2 weeks.

Results are expected to last 3 to 5 years, and the procedure can be repeated whenever required. Some patients may not even need repeated treatments at all.

Depending on how serious your eye bags are, the swelling and bruising can range anywhere from 2 to 3 days to a week.

Both AGNES and Secret RF are gentle on the skin, and it is very rare that one will experience any side effects. However, be prepared for some swelling and mild bruising for the first 2 to 3 days after your session.

Avoid any painkillers, alcohol, smoking, or anything containing Vitamin E before your procedure.

Avoid facials, forceful actions or massages to the face as there will be mild swelling and bruising at the treated areas.

Avoid consuming alcohol, including medications or supplements that may increase your risk of bruising such as Vitamin E, NSAIDS, gingko biloba and fish oil.
